Summary

Instructions for G715–2017, Supplemental Attachment for ACORD Certificate of Insurance 25, is used to supplement the ACORD Form 25, certifying contractor insurance coverage required under AIA agreements. It provides additional space for coverages like General Liability, Workers Compensation, and more, which may not be included on the ACORD certificate. The form also addresses obligations for notification of policy cancellations and insurance coverage post-final payment.

Synopsis. 

AIA Document G715–2017 is intended for use in adopting ACORD Form 25 to certify contractor insurance coverages required under standard form AIA agreements. Because the ACORD certificate does not have space to show all coverages required in standard form AIA agreements, the Supplemental Attachment form should be completed, signed by the contractor’s insurance representative, and attached to the ACORD certificate. For all document details and a record of changes, see the summary »

 

Completing G715–2017.

The Insured’s General Liability, Automobile Liability, Excess Liability, and Workers Compensation and Employers Liability can be shown on the ACORD certificate in the proper boxes. Additional information about the General Liability and Voluntary Compensation should be shown on the Supplemental Attachment. 

The ACORD certificate states that if any of the policies identified in the certificate is canceled before the expiration of the policy, notice will be delivered in accordance with the policy provisions. Many liability insurance policies do not require the insurer to provide such notice of cancelation to parties other than the primary insured. Thus, many standard form AIA agreements such as AIA Document A201®-2017, General Conditions of the Contract for Construction, require the insured party to provide notice to certificate holders within three (3) business days of the date the insured becomes aware of an impending or actual cancelation or expiration of any required insurance. 

Additional Obligations

Section 9.10.2 of AIA Document A201-2017, requires the Contractor to submit to the Architect a certificate evidencing that insurance required by the Contract Documents remains in force after final payment is in effect prior to release of retainage and final payment. The insurance representative should note this requirement when reviewing the Insured’s insurance program. The insurance representative should obtain and review a copy of the Contract to be sure all insurable obligations are noted. 

If additional language has been incorporated into the Contract for Construction, other insurance obligations may be required of the Contractor and should be shown on the ACORD certificate or the Supplemental Attachment.
