AIA Document F704™–2023 is a standard form for reporting the status of maintenance work to a client. It should be used in situations where a maintenance contractor intends to provide a report to a client on the status of maintenance work performed over a period of time. F704-2023 requires the contractor to identify the maintenance agreement or work order contract under which the maintenance work was performed. It also has prompts for the contractor to describe the maintenance work that it performed and to describe any complaints and incidents related to it. F704-2023 may be used to report the status of maintenance work performed under the following agreements and work orders:

Completing F704-2023.
Parties. The names of the parties should be indicated as they appear on the Maintenance Agreement or Work Order Contract.
Status Report Number. The Status Report number should be numbered sequentially in the order the reports are made during the term of the Maintenance Agreement or Work Order Contract.
Status Report Date. The date represents the date the report is written.
Maintenance Agreement or Work Order Contract Description. Insert the date indicated on the cover page of the Maintenance Agreement for which the report applies. If reporting pursuant to a Work Order Contract, insert the date indicated on the cover page of the work order.
Status Report Period. Insert the date or date range for the Maintenance Work covered in the Status Report.
Maintenance Work Description. Insert a detailed description of the Maintenance Work performed during the reporting period, including a description of the building or location where the Maintenance Work was performed.
Reporting Complaints and Incidents. Describe any complaints from third parties and individuals using the Facilities where the Maintenance Work was performed along with any incidents related to the Maintenance Work that might adversely impact the Facilities. The Status Report is for informational purposes only. Submitting the Status Report does not constitute notice required by the Maintenance Agreement or Work Order Contract, and does not constitute an amendment to the Maintenance Agreement or Work Order Contract.
Impacts on Costs or Time. Describe anything that occurred during the reporting period that may or will impact cost or time related to the Maintenance Work. The Status Report is for informational purposes only. Submitting the Status Report does not constitute notice required by the Maintenance Agreement or Work Order Contract, and does not constitute an amendment to the Maintenance Agreement or Work Order Contract.
Attachments or Exhibits. Identify any exhibits, drawings, specifications, documents or other supporting the information included in the Status Report.
Executing the Status Report
The Contractor should sign and date the request.
Notes.
Use of Non-AIA Forms. If a combination of AIA contract documents and non-AIA contract documents is to be used, particular care must be taken to achieve consistency of language and intent among documents.
