Synopsis.
AIA Document F701™–2023 is a standard amendment form and should be used in situations where a maintenance contractor or their client intends to amend a maintenance agreement or work order contract. F701-2023 requires the parties to specifically identify the maintenance agreement or work order contract that is being amended and to describe the nature of the amendment. F701-2023 also has prompts for the parties to insert adjustments to the contractor’s compensation and schedule due to the agreed upon amendments. F701-2023 may be used to amend the following maintenance agreements and work orders:

Completing F701-2023.
Parties. The names of the parties should be indicated as they appear on the Maintenance Agreement.
Amendment Number. The amendment number should be numbered sequentially in the order the amendments are made during the term of the Maintenance Agreement or Work Order Contract.
Amendment Date. The date represents the date the amendment is written.
Maintenance Agreement or Work Order Contract Description. Insert the date indicated on the cover page of the Maintenance Agreement for which the amendment applies. If amending a Work Order Contract, insert the date indicated on the cover page of the work order for which the amendment applies.
Amendment Description. Insert a detailed description of the amendment or reference specific exhibits describing, in detail, the change to be made in the Maintenance Agreement or Work Order Contract by this amendment. Include any drawings, specifications, documents or other supporting data to clarify the scope of the change.
Compensation Amendment. Insert a detailed description of the amendment to the Contractor’s compensation, including the original compensation amount, the net change by previously authorized amendments, the compensation prior to this amendment, the amount of increase or decrease, if any, in the Contractor’s compensation; and the new compensation amount as adjusted by this amendment.
Schedule Amendment. Insert the amount (in days) of the increase or decrease, if any, in the schedule, and the amended date of completion for the Maintenance Work.
Executing the Amendment
Sign the amendment in a manner consistent with that of the Maintenance Agreement. The parties should execute and date the amendment for the date when the Client and Contractor reached an agreement on the change to be made in the Maintenance Agreement or Work Order Contract, including any adjustments in the compensation and schedule.
Notes.
Use of Non-AIA Forms. If a combination of AIA contract documents and non-AIA contract documents is to be used, particular care must be taken to achieve consistency of language and intent among documents.
