When to Use / Purpose.
A C305–2024 may be used to:
receive a consultant’s qualifications as part of the selection process or as a final check before signing a contract; and
review information about a consultant’s business structure, key personnel, honors and awards, project personnel and consultants, software use (e.g., digital design tools, collaboration software, etc.), sustainability credentials, firm history, number of employees, and much more.
Instructions. Completing C305-2024.
General
C305-2024 is not a contract and does not need to be signed by the requester. C305-2024 may be completed at several distinct stages. In many instances, requester may seek a C305-2024 from a consultant, or multiple consultants, as part of an initial selection process before executing a consultant agreement. However, in some cases, a requester will seek a C305-2024 from a consultant immediately after executing a consultant agreement to document more specific information about the consultant it entered a contract with.
Article 1
Submitted To. Insert the name and address of the organization where the C305-2024 will be sent. Other information may be added, such as telephone numbers and electronic addresses.
Project. Insert the name and address of the Project associated with the C305-2024, if known.
Article 1 Firm Information. Provide information pertaining to the consultant firm. If any of the sections in Article 1 do not apply to the consultant, or to the requester’s anticipated project, indicate such in the prompt or delete the section entirely. Section 1.14 asks the consultant to list the commercial and professional liability insurances that the consultant maintains. In many instances, the requestor will ask for more detailed information about insurance held by the consultant in the contract negotiation process or through a request for proposals.
Article 2 Firm’s Capabilities. Provide information pertaining to the firm’s capabilities. If any of the sections in Article 2 do not apply to the consultant’s firm, or to the requester’s anticipated project, indicate such in the prompt or delete the section entirely.
Article 3 Project Team. If C305-2024 is being requested in anticipation of a particular project, the consultant should insert information pertaining to services that will be provided using its own employees in Section 3.1 and information pertaining to services that will be provided using subconsultants in Section 3.2.
Article 4 Project Personnel. If C305-2024 is being requested in anticipation of a particular project, the consultant should insert relevant information requested in this section. In section 4.2., the consultant should provide information about key personnel from the consultant’s office who will be meaningfully involved with the project.
Article 5 References. The consultant should provide professional references from three previous clients with whom the consultant has worked in the past.
Article 6 Representative Projects. Describe five projects that are representative of the consultant’s work. The consultant should use discretion to select projects that are relevant to the potential engagement. For example, if C305-2024 is requested in anticipation of a particular project, the consultant should provide information about similar projects.
Article 7 Attached Documents and Additional Information. In the space provided, reference any documents attached to the C305-2024 form. This could include a marketing brochure prepared by the consultant or photographs of projects representing the consultant’s work. The consultant can also use this space to insert information about additional qualifications not mentioned in earlier articles.
Consultant’s Representation. The C305-2024 should be signed by an individual authorized to represent the consultant’s firm. The consultant’s signature is a representation that, to the best of the consultant’s knowledge, the information provided in the Consultant’s Qualification Statement is true and accurate as of the date the document is signed.
